Bellsprout

You begin to shrink down to two feet tall. Your head turns yellow as you neck gets longer and thinner. The area where your neck connects to your head becomes the top of your head. Your nose vanishes as your lips move to the bottom of your now-bell-shaped head. Your arms turn green and flatten into the shape of leaves. You become impossibly thin, while your toes become roots.
